Patient Forms
Before attending your Initial (FIRST) Appointment, we ask that you complete the Online Intake Form when you make your On-line Booking to streamline your appointment. PLEASE
COMPLETE THE ENTIRE FORM. If you phone in to book your Initial Appointment - or- you are not able to use a computer; we ask that you arrive at your Initial Appointment at least 10-15
minutes early to complete the paper forms in person. **You will be asked to sign a Treatment Consent Form as well as an Insurance Consent Form if you require Health Insurance Direct Billing. This paperwork provides us with your health history and information on your condition. We require your Alberta Health Care Number as well as the Group Policy Number and ID number if you require us to Direct Bill your Insurance Benefits. It is also very important that you supply us with your Email and Address if you have one -- along with a Cell phone number as this enables us to send you Appointment Reminders the day before your appointment.

Initial Patient Consultation
If you arrive early... for your first appointment, please feel free to use our Complimentary Electronic Massage Chair. If you have not completed your Intake forms, please arrive 15 minutes early to complete it before the Consultation and Treatment. In the Initial / New Patient consultation, the doctor will review your health history, discuss your health-related problems, concerns, and potential treatment options. This initial visit is designed for the doctor to learn more about you, your condition, and expectations to determine how chiropractic care can meet your goals. You must express any concerns you have about the style of adjustment you will be most comfortable with if you have a preference. Example - Activator Method instead of Manual Adjustments.

 

Examination
After your initial consultation, the Doctor will perform a complete chiropractic examination testing your reflexes, your ability to turn and bend as well as other standard neurological, orthopedic,
postural, and physical examinations. Nothing will be done in our office without your consent.

Treatments
Patients will receive their first treatment after the consult and exam. This may include spinal adjustments, physical therapy, and/or soft tissue therapies. Treatments can be varied to the patients needs or preferences (e.g. Activator method). Please advise the Doctor of any concerns or requests before your treatment as we want you comfortable and satisfied with your treatment.

 

Follow-up Appointments
When you book follow-up appointments, you select your correct age group with:

Adjustment with Soft tissue Therapy - 20 minute - $70

Post-treatment Advice & Rehabilitation

Prior to leaving, patients may be given advice, and or rehabilitation exercises to be conducted at home. If you are in pain when you first come into our office this may include ice or heat application instructions, avoidance of certain activities or positions, as well as home exercises and/or stretches. Every patient is unique and given advice or a rehab plan specific to them recovering and maintaining a healthy lifestyle.
